Sports Physicals

Students who wish to participate in interscholastic athletics must have a Preparticipation Physical Evaluation (PPE) performed before they can play. PPEs, or sports physicals, are similar to routine physicals in that a medical history and physical exam are completed, and the doctor takes vital signs and checks your child's vision.

The purpose of a PPE is to help ensure that your child is healthy enough to safely participate in sports. Following up on any health issue is a good idea to get your athlete ready for game time.

Access the Preparticipation Physical Evaluation Form from the Florida High School Athletic Association website >

School Physicals

Students in grades K - 12 who are starting school in Florida for the first time must present a record of a physical exam completed within the past 12 months. A specific form is not required, but it is recommended that you use Florida's standardized School Entry Exam Form. It must be completed by a healthcare provider licensed in the U.S. to perform physical exams.

Students transferring to a Florida school from another state can use a comparable form from that state as long as it has been completed within one year.
